Free Dental Sealants for 2nd and 5th Graders

  • REMINDER: San Francisco Department of Public Health (SFDPH) Dental Services will be setting up a mobile clinic at your school this spring to offer FREE dental sealants to 2nd and 5th graders. 
  • Parents / guardians must return active consent forms via interoffice mail to the SFUSD Central Office at 1515 Quintara Street.
